Ward spotlight - Co-producing care with patients and carers

Shelley Ward from the Cygnet Hospital Bierley focused their QI project on developing accessible care plans that would enable greater coproduction of care with patients and carers.

From the start of their QI project, the Shelley Ward team had a very clear idea that they wanted to implement change that would enhance patient voice and engagement with their care. During the coaching period, they developed and implemented a new, accessible care plan template for patients to enhance their input into discussions on their care, complete with a co-produced one-page visual summary for service users of their goal for the coming month and action plan for achieving this. In the video below, Vicki the ward manager takes us through their change idea and the brilliant impact it has already had for staff and patients on the ward.
